JOIN OUR PRACTICE

If you would like to join Honley Surgery please register via one of the ways listed below.

Please note that all new patients aged 5 and over are required to have a new patient check with the practice nurse. The nurse will check your blood pressure, your height and weight and note any current health problems, allergies and regular medications. You can book this once you have been registered.

REGISTER ONLINE

We are using a new online service called 'Register with a GP surgery' that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one. (See next section).

If you are visiting the UK and are wanting to register with the practice please read the following leaflet carefully and fill out the forms located above depending on your age.

OUT OF AREA

From January 2015 all GP practices are able to register patients who live outside of the defined practice area without any obligation to provide home visits.


Out of area registration is voluntary for GP practices meaning patients may be refused registration because they live out of area.


We consider all applications on an individual basis, taking into account your health problems to ensure that it is medically appropriate and practical to register you with our surgery.


To do this we may:

• Ask you or the practice you are currently registered with questions about your health

• Ask you questions about why it is practical for you to attend this practice.


This new arrangement only applies to GP practice and patients who live in England.
