Online Access is only available for patients aged 16 and over, please read the proxy access section regarding children and patients that are unable to manage their own online access.
To apply for Online Access for yourself click the links below to download the Online Access application form & Information leaflet or collect them from Honley Surgery.
Once you have completed the form, hand it in to the surgery along with some photographic identification such as a passport or driving license and proof of your address such as a utility or council tax bill.
Once your form has been processed we will send you a confirmation email or text to advise you to come and collect your log in details from the surgery. You will need to bring some form of photographic identification to pick up your log in details, without this we will not be able to give you your log in details.
PROXY ACCESS
A parent/guardian, carer or family member can apply to have Proxy Access to a patients online records, this may be required for children or patients that are unable to manage their online access themselves due to reasons such as a disability or learning difficulties. The proxy user will then be able to book appointments and order medication online for the patient.
To apply for proxy access please click on the link below to download the form and information leaflet.
Please then bring the completed form to the surgery along with a form of photographic identification for yourself and the patient.
