PHARMACISTS
We have 4 clinical pharmacists who work across The Valleys Primary Care Network. They specialize in structured medication reviews, long term condition advice and medication queries i.e. Side effects and supply issues.
PHLEBOTOMISTS
Honley Surgery has an in house Phlebotomist and a Hospital Phlebotomist visit twice a week to take routine blood samples in addition to the services provided by our nursing and healthcare team.
PERSONALISED CARE TEAM

They work with patients who will benefit from support around improving their health and wellbeing and achieve their self-care goals. They work with patients to identify appropriate organisations and services that are available to help them achieve their goals. They will make onward referrals and organise a team of support to help patients better manage their own care and needs.

HEALTH & WELLBEING COACH
Health and Wellbeing Coaches support people over the age of 18. They work within GP surgeries and help patients to take the to improve how they manage their physical and mental health based on what matters to them. Health and Wellbeing coaches can offer support on a 1-1 or group basis, and also facilitate the setting up of peer to peer groups.
